Localización inusual de un quiste dermoide verdadero en región retroauricular. Reporte de un caso

Resumen


Los quistes dermoides son lesiones del desarrollo poco frecuentes en la región de cabeza y cuello, comprendiendo aproximadamente el 7% de las patologías quísticas de esta zona. Generalmente, se localizan en la línea media debido al atrapamiento de células ectodérmicas y mesodérmicas durante el cierre de los arcos branquiales. Su aparición en la región retroauricular es extremadamente inusual. El objetivo de este artículo es documentar el caso de una paciente femenina de 22 años que acudió al servicio de cirugía maxilofacial de Hospital Antonio Patricio de Alcalá, Cumaná, Venezuela, con un aumento de volumen en la región retroauricular derecha presente desde el nacimiento. A diferencia de la presentación clásica asintomática, la paciente refería dolor asociado a la lesión. Tras la evaluación imagenológica y la biopsia excisional mediante abordaje de ritidectomía modificada, el estudio histopatológico reveló una cavidad quística revestida por epitelio con presencia de anexos cutáneos (glándulas sebáceas), confirmando el diagnóstico de quiste dermoide verdadero. Se discute la importancia del diagnóstico diferencial y la correlación clínica-histológica en ubicaciones atípicas.
