Protocolo bio-digital integrado en implantología: desde la planificación quirúrgica 3D hasta la modulación tisular
Resumen
Introducción: La implantología guiada optimiza la precisión posicional de los implantes de titanio, pero suele ignorar la preparación biológica del lecho receptor. Combinar la odontología fotónica y química transforma el flujo digital pasivo en un protocolo bioactivo integrado. Objetivo: Proponer un protocolo clínico estandarizado que fusione la planificación digital y la impresión tridimensional de guías con fotobiomodulación, terapia fotodinámica antimicrobiana y oxígeno activo. Métodos: Se realizó una revisión exploratoria en Science Direct, Medline, Scientific Electronic Library Online, Biblioteca Virtual en Salud, Scopus y Cochrane (2016-2025) siguiendo las directrices para revisiones sistemáticas. El riesgo de sesgo se evaluó con herramientas específicas para ensayos aleatorizados, no aleatorizados, transversales y de laboratorio; la certeza de la evidencia se midió mediante el sistema de clasificación de recomendaciones. Resultados: Se incluyeron 18 estudios. La terapia fotodinámica (azul de metileno al 0,01%, láser de 660 nanómetros, 6-30 julios por centímetro cuadrado) descontamina el lecho óseo sin citotoxicidad. La fotobiomodulación (660-1064 nanómetros, 2-6 julios por centímetro cuadrado) acelera la osteointegración y mitiga el dolor, activando vías celulares como la proteína morfogenética ósea y regulando al alza el factor de transcripción asociado a enanismo 2 y la fosfatasa alcalina. Los geles de oxígeno activo optimizan el sellado mucoso vía factor inducido por la hipoxia 1-alfa sin estrés oxidativo. Se estructuró un protocolo de cinco fases con desviación de 0,1-0,2 milímetros en la guía quirúrgica. Conclusiones: La simbiosis bio-digital redefine la implantología. Se proponen variables como el cociente de estabilidad del implante y la escala visual analógica para futuros ensayos clínicos aleatorizados.
