Efectos adversos de los medicamentos psicotrópicos en el tratamiento de los trastornos temporomandibulares: una revisión rápida
Resumen
Introducción: Los psicofármacos (antidepresivos, anticonvulsivantes, benzodiazepinas y relajantes musculares de acción central) se emplean en el tratamiento del dolor crónico asociado a los trastornos temporomandibulares (TTM) por su capacidad para modular la sensibilización central, pero sus efectos adversos no han sido sistemáticamente evaluados. Objetivo: Sintetizar la evidencia disponible sobre los efectos adversos de los psicofármacos empleados en el tratamiento de los TTM. Materiales y métodos: Se realizó una revisión rápida en las siguientes bases de datos: PubMed, Google Académico, SCOPUS, COREUK, Dialnet y DOAJ publicados desde el2000; se incluyeron ensayos clínicos aleatorizados que evaluaron los efectos adversos de los psicofármacos empleados en el tratamiento de los TTM. Resultados: Se incluyeron ocho ensayos clínicos; cuatrode ellos informaron explícitamente efectos adversos; solo uno presentó frecuencias de ocurrencia y un estudio describió un método de monitorización. Se informaron los siguientes efectos adversos: boca seca, náuseas, mareos, somnolencia, fatiga, visión borrosa. Conclusión: No se encontró evidencia científica sólida que evalúe el balance riesgo-beneficio del uso beneficio de los psicofármacos, por lo que se recomienda realizar ensayos clínicos que incorporen métodos estructurados de monitorización, especificar la reversibilidad de dichos efectos y realizar seguimiento postratamiento
